Adult Day Health Care of Mad River is a medically based day program for adults who are dependent on others for some of their care and was established in 1985 as a 501(c)3 nonprofit on the campus of Mad River Community Hospital. Although we share a close relationship with the hospital, we have always been a separate entity. Our core mission is to help our participants to remain in their preferred level of care for as long as possible and avoid unnecessary placement in a skilled nursing facility. Eligible participants are enrolled to attend our program on a regular schedule up to five days per week depending on their needs. Enrolled participants have access to a full range of services from our professional staff which include Registered Nurses, Social Workers, Physical, Occupational and Speech Therapists, Activity Coordinators and personal care aides. We also provide transportation to most of our participants to and from their homes. In addition to the direct services that we provide to our participants, caregivers are also provided with essential respite time to recharge and recover from the demands of 24/7 caregiving. These services have historically been paid for by Medi-Cal, the Veteran’s Administration and some long term care insurance plans such as those offered through CalPERS.
The work provided by home caregivers is often unrecognized, exhausting and overwhelming and can lead to depression, financial stress and other health problems. When the caregiver is suffering it is hard to provide appropriate care, and the person they are caring for also suffers. Adult Day Health Care of Mad River is here to help ease that caregiver burden. We serve adults over the age of 18 and provide care for participants with a wide range of diagnoses including dementia, developmental disabilities, and chronic medical conditions such as Parkinson’s disease and strokes.
For families who may not need full time care, we are now able to provide two other respite options. Recently we entered into a partnership with HōttoCare, a company based in the Bay Area which focuses on dementia care through the GUIDE model. GUIDE stands for Guiding an Improved Dementia Experience, and its research based focus is on supporting both the patient and the caregiver to provide comprehensive services. One important component of these services is respite care. Under this plan, the person with dementia can receive up to twenty five days per year of services in our center while the caregiver can focus on their personal care thereby preventing caregiver burnout. This time can be spread out over the course of the year or used in a block if the caregiver has a specific need.
We also have a contract with Partnership Health Plan to provide respite services in our center for those who are covered by this Medi-Cal plan. This plan allows for up to 336 hours of caregiver respite hours per year and has flexibility in how those hours are used. Participants under this plan do not need to complete the full enrollment process for our standard program and can drop in for two to five hours per day on an as needed basis. Approval from Partnership is generally received within 5 days, and attendance can be reserved with only 24 hour notice.
