Our Pathways to Health
Do you have high blood pressure, high cholesterol, diabetes, arthritis, depression, heart disease, or another chronic condition? Would you like to learn techniques for self-management in a supportive and productive setting? The Community Health Alliance of Humboldt-Del Norte will offer many opportunities to participate in “Our Pathways to Health” workshops this year. “Our Pathways to Health” is a FREE chronic disease self-management workshop that was designed to help individuals with chronic conditions (or caregivers for those with chronic conditions) gain self-confidence in their ability to manage their symptoms, self-advocate in medical settings, and develop and achieve mini-health goals in an encouraging and collaborative environment.
Our Pathways to Health is a powerful, rewarding experience. Workshop graduates experience better health, more energy and confidence, less pain and fatigue, improved communication with health care providers, and fewer doctors visits and hospital stays. As a past participant stated, Our Pathways to Health can be a first step towards “living a longer and happier life.”
These supportive, curriculum-based workshops meet once a week for two and a half hours, for a duration of six weeks at various community locations. The free of charge, peer-led program covers topics such as techniques for dealing with frustration, fatigue, pain, and isolation, the role of good nutrition in condition management, and tips for improved communication with health professionals, family, and friends.
